Up to 5,000 firms	50 cents each
Up to 10,000 firms	45 cents each
Up to 15,000 firms	40 cents each
Up to 20,000 firms	35 cents each

Sizes:

No Larger than 8 1/2" x 11" after folding

Paper Weight Limitations:

Two 11" x 17" Sheets Bond - up to 60#
One 11" x 17" Sheet Gloss Text or Cover - up to 80#
Two 11" x 17" Sheets Index or Card Stock - up to 110#

Additional sheets are available at an insert charge of 20 cents per sheet
200 piece minimum
Individual states must be done in entirety
Prepayment required
